If you are on repeat medications, you can request your medication in the following ways:
- Tick the medication(s) you require only, from the list attached to the prescription form –then post it or bring it in to the surgery
- Write down the medication(s) you require, along with your name, address and then bring it in to the surgery or post it.
- Fax your repeat medication(s) request to 01255 207 661 – remember to include your full name and address.
- Order on-line. Please ask at reception if you do not have a password.
You can collect your prescription from the surgery or ask to have it delivered to a pharmacy. Please allow at least 5 working days for the prescription to be issued and signed by a doctor. If you request a repeat prescription too early without reason it may not be issued until its due date, or you may be called to establish a reason. This is to safeguard the patient and the practice from overuse or misuse of medication.
You will also be required to have a medication review done around every 6 months, this is standard practice in ALL UK GP surgeries. This is extremely important to both you and the surgery. It enables us to make sure all your medication(s) is up to date, correct dosages, stop medication(s) or add medication(s) etc. It is also an opportunity for you to discuss any issues you have with any of your medication(s).
